Return & Exchange Policy
At DiscoForms.com our goal is your satisfaction. If you are unsatisfied with your purchase, it may be returned for any reason within 31 days of our shipping date. We accept our garments for return in an unworn (you may try them on for fit), unwashed and unaltered state. Items should be in original condition and packaging with all tags and labels as shipped. Footwear must be clean and in original condition. Please try on your shoes on a clean, dry surface to avoid discoloration to the white soles.

Exchange Instructions:
1. Place a new order online, by phone or by fax prior to packaging your return(s).
2. Write your new Order ID number on a copy of your original receipt or packing slip and include it w/ your returns. (see below for Return Instructions)

1. Mail returns with a copy of your packing slip or receipt to the address below.

Your account will be credited for the returned merchandise upon receipt. There is no re-stocking fee for merchandise returned for exchange. Items returned for refund only will be assessed a 10% re-stocking fee.
Shipping costs are not refundable unless item(s) is defective or incorrect when received. Items returned which have included special order requests, items returned after 31 days of shipment date, or items which have been laundered or altered are not applicable for a refund and will not be returned to sender.
Ship returns via insured mail or carrier. If you use USPS please ask for delivery confirmation. We are not responsible for any delivery exceptions.
To assure proper credit, all returns are subject to inspection.
